PD-specific assessment & goal setting

UPDRS motor subsection evaluation, Timed Up & Go, Berg Balance Scale, 10-metre walk test, freezing of gait questionnaire, and functional reach test. Goals are co-designed with patient and family - e.g., 'walk independently to the local shop,' 'get up from the floor after a fall.'

Amplitude-based movement training

LSVT BIG principles: exaggerated amplitude of all movements to counteract the hypokinetic (underscaled) motor output that defines PD. BIG steps, BIG arm swings, BIG trunk rotation. Training re-calibrates the internal perception of 'normal' movement size to actually-normal kinematics.

External cueing & gait strategies

Visual cues (stripes on floor), auditory cues (metronome, counting), and attentional strategies ("step over the line") to bypass the deficient basal ganglia automaticity circuit and engage the frontal cortex for gait initiation - specifically effective for freezing episodes.

Balance, falls prevention & flexibility

Reactive and anticipatory postural adjustments, rotational trunk movement training (trunk rigidity is an under-treated contributor to falls), floor transfer drills ("how to get up safely"), and flexibility work for the characteristic flexed posture.
